I'm a primary care nurse practitioner managing a panel with a high prevalence of type 2 diabetes, and I'm frustrated that despite our best efforts with medication adjustments and standard dietary advice, many of my patients struggle with glycemic control and seem defeated by the complexity of self-management. I feel my current approach is too reactive, focused on treating numbers rather than empowering sustainable lifestyle change, especially for patients dealing with food insecurity or limited health literacy. For other providers in similar settings, what patient-centered strategies or tools have you successfully integrated into your type 2 diabetes management to improve engagement and outcomes? How do you structure visits to collaboratively set achievable goals, and what role have continuous glucose monitors or group visits played in your practice for non-insulin dependent patients?
