What are the craziest film set hidden stories you know?

From my legal work in entertainment, I've heard some absolutely wild film set hidden stories that never make it to the public. The contract disputes, the hidden clauses, the secret payments - it's a whole world behind the scenes.
One production I worked on had to hide the fact that their lead actor was actually in rehab during filming. They used a body double and CGI for most scenes and claimed the actor was "preparing for the role" in isolation. The insurance company nearly sued when they found out.
What film set hidden stories have you heard that show the reality behind the glamour?
The rehab story is wild but believable. I've seen similar film set hidden stories. On one production, the lead actress was pregnant but didn't want anyone to know because it would affect her action star" image. They used clever costuming, body doubles, and strategic shooting angles to hide it for months. The crew all knew but had to sign NDAs. She gave birth two weeks after filming wrapped.
In post-production, we sometimes discover film set hidden stories through the footage. I worked on a film where two supporting actors clearly hated each other - you could see it in their body language in every scene together. The director had to shoot them separately whenever possible and we had to edit around their lack of chemistry. The press tour was awkward because they had to pretend to be friends.

Or the action film where a stunt went seriously wrong and someone was injured, but the production covered it up to avoid bad publicity and insurance claims. As a writer on set, you hear all the film set hidden stories. One that stuck with me was a director who was secretly filming two different endings with different actors as the villain. Not even the studio knew. He shot one version with Actor A as the traitor and another with Actor B. He didn't decide which to use until the final edit. The actors didn't know they were potentially playing the villain until the movie came out.
