How important is game documentation in your projects?

I used to think documentation was something only big studios needed to worry about. But I've come to appreciate the game documentation importance even for small indie projects.

The game documentation importance became clear to me when I went back to work on an old project after six months. I couldn't remember how certain systems worked or why I made certain design decisions. Good documentation would have saved me so much time.

How do you approach game documentation importance in your projects? I'm interested in what you document, what tools you use, and how you balance documentation time against development time.
