I keep a one page symptom tracker for visits It lists the symptom when it started how long it lasts the triggers you notice and what helps or makes it worse I also note any medications and doses last labs and a quick timeline of events I print it out or keep a note on my phone so I can hand it to the doctor without fumbling I use a zero to ten scale for pain or intensity and I flag red flags like fever above 38 C or sudden weakness

I keep a running note with the start date and time the symptom began the duration and what changes it I also jot down current meds allergies and the last test results so the PCP can see the full picture without digging
