I've been struggling with chronic sinusitis for years, characterized by constant facial pressure, congestion, and recurrent infections that antibiotics only temporarily relieve. My ENT has recommended a CT scan and is discussing options ranging from long-term corticosteroid sprays to possibly endoscopic sinus surgery if structural issues are found. I'm hesitant to jump to surgery but also exhausted from the relentless symptoms impacting my sleep and concentration. For others who have been down this path, what treatment sequence finally provided you with lasting relief? If you opted for surgery, what was your recovery experience like and did it truly resolve the underlying problem? For those who managed it medically, what combination of medications, irrigations, or lifestyle changes made a sustainable difference in reducing the frequency and severity of your flares?

A common starting point is a structured medical optimization for 6–8 weeks: daily nasal corticosteroid spray, saline rinses twice a day (neti pot or squeeze bottle with distilled water), and lots of hydration. Use antihistamines if you have allergic rhinitis. Track infections/flares with a simple diary. If you’re still miserable, imaging showing polyps or blockages helps discuss next steps like targeted therapy or surgery.
From my viewpoint, two paths: medical-first vs early surgery. I tried aggressive medical therapy for several months; it helped some but not enough. We then weighed endoscopic sinus surgery. For many with nasal polyps or recurrent drainage, surgery can improve drainage and reduce need for steroids, but it’s not cure-all; you’ll still need to maintain nasal sprays and hygiene afterward to prevent recurrence.
Know about newer options: biologics such as dupilumab can reduce polyp size and inflammatory burden in patients with chronic rhinosinusitis with nasal polyps; several trials show symptom relief; cost and access are the tradeoffs; plan ahead with insurance.
