Full Version: What non-surgical options help chronic sinusitis management?

I've been dealing with chronic sinusitis for years, with constant facial pressure, congestion, and recurring infections that antibiotics only temporarily fix. My ENT has suggested I might be a candidate for sinus surgery after CT scans showed significant blockage, but I'm hesitant and want to explore all my management options first. I'm currently doing daily saline rinses and using a steroid nasal spray, but the relief is minimal. For others who have navigated this, what non-surgical approaches provided you with meaningful, long-term relief? Did you find success with allergy testing and treatment, longer courses of specific medications, or other therapies like balloon sinuplasty? I'm trying to understand what truly works to improve quality of life before considering an invasive procedure.
You're not alone. For many with chronic sinusitis, consistent non-surgical management can offer real relief. Start with daily isotonic saline irrigations (2–3 times per day if possible) and a daily intranasal steroid spray. Add a humidifier at home and try to minimize environmental irritants like smoke and strong fumes. If allergies might be driving symptoms, allergy testing followed by targeted treatment or immunotherapy can reduce episodes over time.
Balloon sinuplasty can be a good option for some people who have persistent blockage, but it's not a universal fix. It preserves more tissue than full open surgery and often has a quicker recovery, but success depends on the extent of disease. Discuss your surgeon’s track record and what your CT shows to gauge likely benefit.
Non-drug options can also help. Some people benefit from nasal antihistamines added to nasal steroids if allergies are a trigger. Be mindful of overusing decongestants and aim for a balanced approach. If nasal polyps or chronic infections are part of your pattern, your ENT might discuss targeted therapy or maintenance plans that don’t involve long-term antibiotics.
Questions to ask your ENT before deciding on surgery: 1) Given my imaging, do you expect balloon sinuplasty or conventional endoscopic sinus surgery to offer the best long‑term relief? 2) What’s the plan if medical therapy alone isn’t enough? 3) Are allergy testing or immunotherapy appropriate for me? 4) What are the potential risks and the expected recovery timeline for each option? 5) What follow-up schedule and milestones should I watch for?
If you’d like, I can help sketch a simple two‑month plan to discuss with your doctor—start with optimizing nasal meds and saline routines, then consider allergy testing or procedural options if symptoms persist. I can also help you keep a symptom/flare diary to bring to visits.
